Dear [Hiring manager’s name],
I am applying for the Clinical Pharmacist position in your oncology unit. I am registered with the Telangana State Pharmacy Council, hold a Pharm.D, and work as a clinical pharmacist on the medicine and oncology wards of a 500-bed hospital.
Each day I review around sixty inpatient medication charts; in our last audit, doctors accepted about 85% of my interventions. I verify chemotherapy orders against protocols with the oncology team before compounding, and I was part of the team that raised medication reconciliation coverage for high-risk patients on my wards from 55% to 90%.
I take pharmacovigilance seriously: I report adverse drug reactions to our hospital committee and to PvPI, and I have trained ward nurses on what to report. Before clinical work I dispensed around 250 outpatient prescriptions a day and maintained Schedule H1 records, so I understand both sides of the pharmacy.
